Bexley Health Neighbourhood Care (BHNC), a not-for-profit NHS community interest company serving 25 GP practices and more than 240,000 patients in South East London, coordinates care across 600 staff and multiple sites including temporary COVID-19 sites and care homes. BHNC’s legacy systems and paper-based processes made it hard to see staff, facilities and inventory availability, produced slow and inaccurate reporting, and created manual, inefficient care coordination that was exacerbated by the COVID-19 crisis.
Using Appian’s low-code automation platform, BHNC deployed a Resource and Care Coordination solution in one week that provides a 24x7 scalable unified dashboard, mobile digitised patient records, daily staff health check-ins, incident reporting and a Central Command hub for real-time supply-and-demand visibility. The system improved resource planning, prioritisation and cross-team collaboration, gave practice managers local and regional operational visibility, and helped maximize capacity and streamline patient journeys from hospital to home.
