Geisinger Health System, a large physician-led organization serving more than two million residents in Pennsylvania, sought to lower all-cause 30-day hospital readmissions among Medicare Advantage patients. Traditional telephone-based case management identified care gaps but was staff-intensive and hard to scale, so Geisinger partnered with AMC Health to deploy an interactive voice response (IVR) telehealth program to automate routine post-discharge calls.

Geisinger and AMC Health jointly implemented the IVR within Geisinger’s Medical Home case management model, expanding from two clinics to 42 sites and enrolling over 5,800 participants. The AMC Health IVR plus traditional case management reduced the odds of 30-day readmission by 44% versus case management alone (n = 875, p < 0.0004), and the partners are extending the telehealth approach to hypertension and e-diabetes programs.
