Healthpartners Medical Group, a large nonprofit integrated health care organization serving over 1.5 million members, sought to improve blood pressure control among patients with hypertension. To tackle this, they partnered with AMC Health to implement a home blood pressure telemonitoring program paired with pharmacist case management.

AMC Health supplied home BP monitors that transmitted readings to pharmacist case managers, enabling proactive medication adjustments and telehealth follow-up. After 12 months the telemonitoring group saw an average reduction of 11/6 mm Hg and 71.2% of patients achieved controlled blood pressure versus 52.8% with usual care; improvements persisted six months after the intervention and patients reported greater satisfaction, confidence, and adherence.
