Geisinger Study Demonstrates Reduced Hospital Readmissions and Cost of Care

Geisinger Health System, through its Geisinger Health Plan, sought to reduce utilization and costs for members with heart failure by detecting early deterioration and preventing hospitalizations. To extend its longstanding case management program, Geisinger partnered with AMC Health to deploy remote monitoring of weight and self‑reported symptoms (daily weight telemonitoring with IVR calls) threaded into existing complex care management workflows.

AMC Health’s remote monitoring solution was integrated with Geisinger’s care management, and a within‑person study of 541 members showed a 23% reduced probability of hospitalization and a 44% reduced probability of readmission versus care management alone. The program produced significant reductions in utilization and costs (p<0.01), with average relative cost savings of 11.3% ($2,592 per patient per year) and a 3.3:1 return on investment for AMC Health’s telemonitoring service.
