AMC Health’s Clinical Decision Support Tools Helped HHC Nurses and Patients Lower HbA1c by 1.8 Points¹

New York City Health and Hospitals Corporation (NYCHHC), the nation’s largest municipal integrated delivery system, was struggling to improve chronic disease outcomes — many diabetes patients weren’t reaching an HbA1c below 7%, blood pressure control was inadequate, and ER and hospital use remained high. AMC Health partnered with NYCHHC, deploying its House Calls Telehealth Program and CareConsole® remote monitoring and clinical decision support tools to tackle poor adherence and gaps in self-management.

Using AMC Health’s telemonitoring (daily blood glucose and blood pressure) with NYCHHC nurse coaching and physician medication adjustments, nearly 700 patients achieved substantial improvements: 81% had sustained glycemic control with an average HbA1c reduction of 1.8 percentage points; 66% of hypertensive patients lowered diastolic BP by 5 mmHg (≈21% reduced cardiac event risk, 15% reduced stroke risk); ER use declined 47% and hospital admissions dropped 38%.
