Innovative technology partnership is powering a growing cloud-connected community health network

South Western Sydney Primary Health Network (SWSPHN) faced the common healthcare challenge of fragmented patient data scattered across numerous disparate systems, which hindered timely information sharing between primary care providers and hospitals. This lack of interoperability made it difficult for clinicians to get a complete picture of a patient's health, leading to potential inefficiencies and delays in care. To address this, SWSPHN partnered with Altera Digital Health to implement a solution.

Altera Digital Health implemented its dbMotion Solution, a cloud-based interoperability platform hosted on Microsoft Azure. This system, known as Project iRAD, aggregates and harmonizes patient-consented data from various clinical systems, delivering a unified, actionable view of a patient's record at the point of care. The results include over 100 connected sites, more than 20,000 patient consents, and tangible improvements in care coordination, such as faster diagnosis and treatment, reduced duplicate testing, and more efficient management of patients with chronic conditions.
