Case Study: King’s College Hospital NHS Foundation Trust achieves near-100% documentation of DNACPR and TEP with Altera Digital Health

Improving clear, consistent documentation of treatment escalation plans

King’s College Hospital NHS Foundation Trust, one of London's largest teaching hospitals, needed to improve its documentation of clinical decisions regarding resuscitation and treatment escalation (DNACPR and TEP). With a paper-based system, the rate of completed forms for new admissions was below 20%, presenting a significant patient safety and compliance challenge. To address this, the Trust partnered with vendor Altera Digital Health.

Altera Digital Health implemented an electronic solution using the Allscripts Sunrise platform to create a unified, intelligent order set. This tool linked the resuscitation status order with the required documentation, ensuring completion. Following implementation, the rate of documented DNACPR and TEP for monthly admissions surged from 18% to nearly 100%. This empowered clinicians, improved multidisciplinary collaboration, and prevented inappropriate interventions for patients nearing the end of their lives.
