Rochester Regional Health, a nonprofit health system using multiple disparate EHR systems, faced the challenge of creating a unified view of patient data across its entire care network. To solve this, they worked with the vendor Altera Digital Health to implement the dbMotion Solution platform, which facilitates connectivity and data aggregation.

The solution implemented by Altera Digital Health created a longitudinal patient record by semantically harmonizing data from all internal systems and external health information exchanges. This enabled clinicians to get a complete picture of a patient's history, which significantly improved care coordination. As a result, one initiative for diabetic patients reduced the number of those with uncontrolled blood sugar by 14% and drastically cut the average time to get patients' blood sugar to target.
