Case Study: Holyoke Medical Center improves care coordination and reduces readmissions with Altera Digital Health

A new vision for comprehensive care coordination

Holyoke Medical Center, a community hospital, faced the challenge of disconnected inpatient and outpatient case management. This created siloes and communication gaps that risked patient care continuity and endangered participation in CMS value-based programs. To address this, they partnered with Altera Digital Health to implement a unified care coordination strategy using solutions including Allscripts Care Director, Allscripts Care Management, and CarePort.

Altera Digital Health provided an integrated technology platform to break down departmental siloes and enable seamless information sharing across the care continuum. The phased implementation allowed for a redesigned, patient-centered discharge process. The results were significant and rapid, including a reduction in readmission rates, a jump in patient satisfaction scores from 88.5% to 92.3%, and a major improvement in care transition scores as measured by Press Ganey.
